Scoring notes

Score the best response observed. If a component cannot be tested (intubation, periorbital swelling), document it as not testable — e.g. VT for an intubated patient — rather than scoring 1.

Trend component scores over time; a falling motor score is the strongest single predictor of deterioration.

Educational use only. GCS scoring supports assessment and trending; airway and escalation decisions are provider-directed. This material supports nursing education and exam review. It is not medical advice and is not a substitute for clinical judgment, institutional policy, or medical direction. Always follow facility protocols and current provider orders.
